Screening
System Order Process…
- Complete the
information below and proceed to the check out page.
- This
is an authorization only. Billing will
occur when the screening unit is shipped.
- A
$200.00 authorization of your credit card
is required in order to cover the screening
fee and shipping costs. Your credit card will be
adjusted accordingly upon return of the screening
unit.
- Actual
screening price is $130.00 for one night's screening. Should you feel that your first screening was not an indication of your average sleep patterns and you wish to run the screening another night(s) the charge will be $25 each night.
- Shipping
charges range from $25 to $50, based on round trip, related to your geographical location. The unit is delivered
to you by UPS second day air. A UPS second day
return label is supplied along with return instructions.
You must follow the shipping instructions and
screening time lines to ensure accurate recovery
of your data.
- Based
on your mailing address, we allow seven business
days from our shipping date for you to complete
the test and return the unit to us. You maybe
assessed $15/day after the seven day period,
until American Sleep Systems receives the returned
kit. If the kit is not returned within 30 days,
the assumption will be that the kit will not
be returned. At that time, an additional charge
of $760 will be charged to you on-file credit
card. If the Oximeter Kit is returned after the
30 day period, a refund of the balance will be
reimbursed after all obligations have been met.
- Additional
charges will apply for multi person testing (up
to 3 tests). Each additional screening report
will receive a charge of $50.
- American
Sleep Systems will contact you within 24 to 48 hours,
upon receipt of your order, for scheduling.
